The service
The United Contact Lens portal is an ordering service for licensed eye-care practitioners and their practices. It is professional equipment, not a consumer store: contact lenses are prescription devices, and by using the portal you confirm you are ordering in the course of professional practice under a valid prescriber's authority.
Accounts and registration
Practices register once and every registration is reviewed before activation. You agree that the information you register is accurate and kept current. At launch each practice has one sign-in; you are responsible for orders placed under it. Sign-in is by single-use email link, so access to the registered email address is access to the account.
Orders and production
Lenses are made to order. Submitting an order starts production immediately: there is no review queue between you and the lab, and the review step in the order form is your check that the parameters are what you intend. If you discover an error after submitting, call the lab at 1-800-446-1666 right away. If something on an order needs checking, the lab calls the prescribing doctor directly.
Prescriptions and professional responsibility
The prescribing doctor named on each order is responsible for the prescription, the fit, and the patient relationship. Lenses are manufactured to the parameters submitted; judging their clinical suitability for the patient remains the practitioner's responsibility.
Pricing and billing
Prices are shown while you order, before you submit. Orders are grouped onto one statement per calendar month, finalized on the first of the following month, and charged automatically to the payment method on file. Paying by bank account (ACH debit) earns a 2% discount on each line of the statement. A failed bank debit is retried; if a statement remains unpaid more than 45 days past its date, ordering pauses until the balance clears. Questions about a charge go to the lab at 1-800-446-1666.
Warranty
Warranty is part of the purchase type. A fitting lens (warranted single) carries a 30-day warranty. Replacement series and other non-warranted purchase types carry no warranty except where the law provides one that cannot be excluded. Warranty claims are handled by the lab by phone.

Acceptable use
You agree not to misuse the portal: no attempts to access other accounts or data, no automated scraping, and no use of the service outside professional lens ordering.
Governing law and changes
